About the Author

Michael L. Kochman, MD, FACP is Professor of Medicine in the Gastroenterology Division at the University of Pennsylvania Medical School and Hospital of the University of Pennsylvania. He is the Endoscopy Training Director and Co-Director of the Gastrointestinal Oncology Program. Dr. Kochman is a graduate of Northwestern University (1982) and the University of Illinois Medical School at Chicago (1986).

At the University of Pennsylvania, he has served on various committees including the Physicians Billing Oversight Committee and Departmental Review Committees. Within the GI division he has served as Fellowship Chairman and has received a number of teaching awards including the Sid Cohen, MD award for the education of fellows. Dr. Kochman has served many local and national societies in a variety of positions. Currently he is President of the Delaware Society for Gastrointestinal Endoscopy and is the Program Chairman for the Pennsylvania State Gastroenterology Society. His major national commitments are to the American Gastroenterological Association (AGA) and the American Society for Gastrointestinal Endoscopy (ASGE). He has served the AGA on the Education Committee and the Program Committee and is currently a member of the Clinical Practice Committee. For the ASGE, Dr. Kochman has served on the Post-Graduate Education Committee, the EUS SIG and is currently a member of the Research Committee and the Program Committee.

Dr. Kochman is the Chairman of the Editorial Board of Gastrointestinal Endoscopy, and is coeditor for Techniques in Gastrointestinal Endoscopy and the Yearbook of Gastroenterology. He also serves on the editorial boards and as a reviewer for a number of journals including Annals of Internal Medicine and Gastroenterology. Good Book!5
"Comprehensive' and "Practical" are the proper words to describe this book.

In contrast to other pain medicine text books, it starts with "General Considerations" in Part I which provides very important and hard to find information such as "Organization of Inpatient Pain Service", "Organization of Pain Services for Chronic and Cancer Pain', "Organization of Pain Management Services in Children" which are instrumental for any pain practices in learning and following appropriate guidelines in establishing and providing such services.

In Part II, the "Basic Consideration" touches on Pain Pathways & Mechanisms, Anatomy and Physiology, Neurochemistry in a rather focused way. Two short Chapters with one on "Pharmacological Management of Pain", one on "Biopsychosocial Evaluation & Treatment of Chronic Pain" are refreshing and informative.

In Part III, the "Clinical Consideration", provides Chapters on "Acute Pain Syndromes", "Pain During Pregnancy", "Phantom Pain", "Visceral Pain", "Cancer Pain", "Pain in Selected Neurological Disorders", "Geriatric Pain", "Pain in Children". I find these chapters helpful in setting up guidelines for the management of acute pain, cancer pain, pediatric pain and pain in pregnant women. E.g, I had tough time during my Pain Management Fellowship training in finding the topic of "pain management in pregnancy" from other pain management text books. The discussion on post op pain management is quite thorough which helped me quite a bit during my Pain Fellowship Acute Pain Service.

In Part IV, the section of "Evaluation & Investigation" includes chapters on taking history, performing basic exams including focused neurological and musculoskletal exams which are practical and useful. However, the "Radiology" chapter needs more spine images since spinal pain from neck to low back is what we see most from chronic pain population. I was surprised to find that it does not have a cervical MRI in this chapter. The chapter on `Differential Neural Blockade" is informative, although the clinical utility of these blocks in clinical practice is questionable.

Part V consists of three sections, A, B, C respectively.

Section A includes chapters on "Pharmacological Techniques", "Pharmacokinetics of Analgesics" and "Neuropsychiatric Drug use in Pain Management" which are well written. Among them, the discussion on opioid analgesics is very useful in clinical practice, especially in the settings of management of acute pain, based on my own personal experience.

Section B contains chapters on procedural approaches of commonly performed interventional procedures including from head to toe peripheral nerve blocks, epidural nerve blocks and sympathetic nerve blocks. Additionally, it has a chapter on "Organization of Nerve Block Facility" that contains instrumental information on personnel, equipment, supplies, documentations, which is extremely useful for newly established pain clinic that plans to provide interventional services.

Section C, the "Special Techniques" that covers varieties of individual interventional arena such as "Epidural Steroid Injection", "Facet Sydromes and Block", "Spinal Opioid for Acute Pain", "Cryoneurolysis & Radiofrequency Lesioning", "Spinal Cord Stimulation" , "Implanted Drug Delivery Systems" , "Surgical Techniques in Pain Management", "Alternative Pain Medicine" and even "Acupuncture", etc. The chapter on "Spinal Opioid for Acute Pain" helped me tremendously during my Fellowship training while on Acute Pain Services at Univ. of Michigan Health System, managing patients on patient-controlled epidural opioid infusion for post-op pain control. Some of the chapters, e.g., the "Epidural Steroid Injection" can be significantly improved with inclusion of more fluoroscopic picture demonstrating anatomic locations of needle placement, contrast spread pattern, etc. The Chapter on "Radiofrequency Lesioning" needs more illustration with pictures showing needle placement as one can not learn these procedures from reading procedural descriptions. Good quality fluoroscopic pictures work way better than words.

Part VI through Part VIII summarizes outcomes, efficacy and complications of pain management in acute pain, chronic pain, neuropathic pain and cancer pain. It also includes discussion of pain management in emergency room, home care and palliative medicine. Finally, it ends with discussion of pain management of future.

In summary, this is a comprehensive, practical text book of pain management. It is an excellent resource for physicians to gain insight into areas pertinent to pain management practice, such as how to set up pain clinic, how to organize acute pain service, how to organize a procedural facility, etc. I personally benefited from reading it in preparing my Board Examination in the American Board of Interventional Pain Physicians(Part II)(Fellow, Interventional Pain Practice, World Institute of Pain).

A must for the pain specialist5
This book is in its third edition. The book is keeping up with the rapidly changing specialty of the Practice of Pain. The current edition has been expanded and extensively revised. The approach is "practical". A must for the practicing Pain Specialist and helpful for the pain patient to understand the diagnostic and therapeutic procedures which are well illustrated.

A good first text on Pain Management...4
This is a very good textbook on invasive pain management techniques, and their utility in clinical practice. The book reads well, is reasonably well illustrated, and includes tables of studies of techniques and their success rates. Additional discussion of the non-invasive medical and physiotherapeutic techniques of pain control for specific disorders are included. The book is multi-authored, with sections by Dr. P. Prithvi Raj, a noted expert in the field. This was the first textbook of Pain Management I purchased for my own clinical practice and it served me well. A new edition is due for 1999. I also would recommend the superb and scholarly Monograph, "Current Review of Pain" edited by the same author. This latter series summarizes the latest developments in the basic sciences and the clinical sciences in Pain Management, is very well illustrated, and the author has gone to the trouble of including an annotated bibliography of the voluminous medical and scientific literature on Pain, enabling the researcher and practitioner to select those articles from Medline most useful to his or her research and/or clinical endeavors.

Importantly, there is no relation between clinical severity, as measured by radiographic changes, and the presence and severity of joint pain. Patients with severe joint pain may have normal joint architecture as determined by X-ray, whereas patients with considerable evidence of joint remodelling may not have significant joint pain. Treatments for osteoarthritic joint pain include non-steroidal anti-inflammatory compounds, exercise, corrective shoes and surgical intervention. There remains a critical need for improved control of joint pain in osteoarthritis.

A life changing book!5
To have the ability to write a book like this and make it credible you are required to be have

a) lived a life like Andrew Griffiths
b) have the balls to tell people the truth and
c) chunk it down so everyone (and I mean everyone) can get something out of it!

This is a book that changes lives. If you want the same old, same old, life, then dont go near this book.

If you want to know how to dramatically increase the amount you suck out of life, then run (dont walk) to get 101 Ways to Have a Business and a Life

Should've Been Called "Every Entrepreneur's Bible!"5
This weekend I picked up three books on time management at Borders...Two of them I've heard of before, written by quite well known authors and business experts.

The third one I grabbed at the last moment before I headed to the check-out, and I am so glad I did!

Andrew Griffith's "101 Ways to Have a Business and a Life" is an amazing read! It's not just a time management book. It should've been called "Every Entrepeneur's Bible!"

It covers everything from managing your time and money to managing your relationships, health, employees, vendors, customers, family, kids, and much much more...I swallowed it in about 3 hours (ok, I skipped some section, but that's how I always read anyway).

The other two books were great, but this one stood out for one big reason: it was based on the author's true story of how his business life took a toll on his relationships and his health, and how he made a decision to "wake up" and realize that his business is "slowly killing" him.

Personal story - that's what's unique about this book, and makes his message so much more compelling. Andrew is also being very candid - he shares the things that most people are embarrased to admit even to themselves, and offers simple solutions.

The book is very easy to read, split into 1-2 page segments, just right for busy entrepreneurs.

I think the book cover doesn't do it justice...I'd love to see Andrew's photo on the front cover!

Go to your local book store and get it today!

P.S. If you're a life coach working with business owners, this is a great book to create your own coaching program around....You can even partner with the author to coach people based on his ideas!

Presenting to Win is an excellent tool4
This book is an excellent tool. It focuses very specifically on effectively creating a business presentation. The tips were valuable to be and I have been creating presentations for several years. It will also be very easy to reference in the future.

Homerun after homerun after homerun ...4
This book is about making CONSCIOUS decisions for balancing the story board itself for hard facts, visual, ethical and psychological aspects, politicily correctness etc. but of course doesnt stop there.

The book shows and discusses which elements you need to convey your story and why you use certain presentation technics over others to achieve your goals.

The book is devided in 14 chapters. Each chapter is focused on either

a) How to create or develop your basic story or on
b) How to enhance it

(by using the described technics and its implications and reactons it will provoke).

What makes this book standing out is the careful analyzation of the aspects that came into play when giving an presentation.

That obviously includes the analytical skills itself but also the time and effort to explicitely mention and discuss (dis)advantages of each element.


The carefully chosen presentation samples will be disassembled throughout the book and taken apart into its peaces, analyzed, explained and put back together.

Where required, the example will be (dis)assembled several times to bring the points across.

Its the analysation of those presentations and its aspects to a granular level and putting the gained knowledge into a conscious presentation creation process that make the book so valuable.

Most books tell you just how to use software to make graphics etc. but this book tells you what you have to present to your adience to actually win them over.

The fact that the many aspects are explicitely explained helps you visualize the options you have at your disposal and the reason why you chose one presentation form over another.

While this book focuses on presentations that show off your assets and the art of persuasion. There is also a companion book "In the line of Fire" which focuses more on the defense to hardball questions.

I do also want to recommend a third book - "Dan Roams: Back of the Napkin" which focuses more on the technical aspects of how to find your story, and a strong focus on visualizing it fool prove and providing rock solid hard facts that wont be beaten.

What Jerry*s books does express very well is the fact that giving a presentation is like being an athlet.
You will have to exercise "verbalize" regularly to be in top form when it counts.

You will never present the same way again...and your audiece will thank you5
Jerry Weissman is one of those rare people who has written an authoritative sounding book about how to present and has the real experience and background to justify every claim he makes.

The book starts with the premise that the presenter must focus on the audience and that he must make them focus on him. He must understand the mental point they are at (Point A) and moves them to Point B. He must understand what is in it for them (WIIFY) and constantly use it as he constructs every slide to walk them to Point B. He must also understand the setting of the audience, and his main points of argument. Finally, he must tie those points together with a flow structure that fits his argument.

That's the first half of the book and as someone who has through some awful presentations, I can only wish reading this book were the equivalent of a driver's license for public speakers.

The back half of the book draws on his background in television and employs standard cinematic techniques to improve the appearance of PowerPoint. It's easy to overlook this part, but it makes a huge difference as well.

I've now had a chance to see people who have used these techniques for years present, and it makes a huge difference. I have also seen someone present in a tough situation using these techniques for the first time. This person is level-headed and not given to fads. His comment? "I wish I had run to Jerry's book ten years earlier."

If you speak in public, this is the one book you have to read, and re-read. It is common sensical, based in fact, and surprisingly intuitive.

Essentail reading for Forex Dealers5
I am a veteran forex dealer and in-house trainer of our dealers; this book is required reading. From the opening quote in the introduction of the book you are introduced to the concept that "psychology" is everything in dealing. Believe it! Though the book provides no canned solutions to dealing, it does give you the essential foundations to begin to build your own dealing models.

A good dissertation may be, but not a good trading book for sure2
If you are interested in the result of a survey done through over 300 FX traders in 5 countries or over 200 bibliographies for your own thesis, you will be absolutely delighted. If you want to sharpen your trading edge or simply to have fun (like what Fiasco, Pit Bull, Liar's Poker and Wall Street Meat do), you will be very disappointed.

I would like to give you an example. In page 154 the author listed a table of importance ratings of successful trader characteristics (n=291, 1=unimportant, 4=very important) of 23 items ranging from Quick Reaction Time (mean=3.71, sd=0.50) & Discipline (mean=3.65, sd=0.55) to Computer Literacy (mean=2.54, sd=0.78) & Social Skills (mean=2.52, sd=0.74). Some might find it helpful. As a veteran pro FX trader and trading book lover who read to sharpen my edge in one of the most competitive market, I dont.

Perhaps the author had just tried too hard to post the findings of his survey into a book that the chapters and thus content are quite disoriented (I strongly suggest you to take a look of the content page available on Amazon). Sorry to say that there are plenty of much better trading psychology books in the market (though not dedicated to FX, but still relevant and applicable). IMHO, "Beyond Greed and Fear: Understanding Behavioral Finance and the Psychology of Investing" by Hersh Shefrin and "The psychology of Finance by Lars Tvede" should be better choices.

p.s. I had overlooked the fact that the two five star reviews (this should be the third review of this book) were written by two one-review-only readers. Shame on me!

A Must Read for Any Trader or Investor5
This book is valuable to any person trading any market. It enforces the thought that you bring yourself to the market, be it currencies, futures, stocks, or bonds. A person is the product of their environment, culture, parents, education and experiences which influences their perception of the markets. It is very difficult to observe what the market is doing without observing what it is doing to you. All markets are a mind game.

Too little2
Goes a mile wide and an inch deep. I would rather the same book but each chapter by an expert in that area with a lesson/example.

Easy and Substantial5
I find this book very easy to read. It is fraught with valuable, profound, substantial insights. It will rapidly acquaintance people who are not yet familar with MBA topics and it will solidify the knowledge of veterans in these issues. So, either way you win. Also, even though this book is intended mainly for service providers (doctors, lawyers), it can be easily extrapolated to any managerial work at whatever stage. Also serves as great inspiration if you ever thought about doing your MBA5
The hidden gem in this book, for those of you who don't have formal business education and always wondered whether the MBA was attainable for you, is that this book actually serves a wonderful second purpose. MBA in a Day allows you to "kick the tires", giving you a high-level overview of what you 'should' expect to cover during MBA study. Having read this book, I found the inspiration to learn more; as such I have enrolled in the MBA program where I already completed a masters in IT.

My only issue with MBA in a Day is that I would have loved to have seen a chapter on Entrepreneurship-- which would be relevant to certain MBA-wannabes (like me), as well as the existing professionals (the main audience of the book) hoping to launch their own private practices.

Neverthelesss, Dr. Stralser presents a wide variety of topics (including HR, Organizational Behaviour, Leadership, Ethics, Accounting/Finance, Economics, Marketing, Project Management, IS and Quality Management) in such an approachable way, making seemingly daunting subject matter easy to digest and appreciate.

A definite buy, I am glad I picked up this book.... if you are thinking about doing your MBA, you should too.
